URGENT NOTICE  
December 4, 2009
 Dear Member or Payee:
 
                     We are writing to you because of a recent incident regarding the potential theft of certain confidential information from the Oklahoma Firefighters Pension and Retirement System (the "OFP").  An unauthorized person may have stolen confidential documents from the OFP that may contain personal information, including social security numbers.  The OFP is currently working diligently with law enforcement to retrieve these documents, and although there is no evidence that an unauthorized person has obtained your personal information and is using it, the OFP recommends you take certain steps to protect yourself in the event your personal information has been compromised. 
 
    First, keep an eye out for any suspicious activity by routinely monitoring: (1) your financial accounts and billing statements regularly, looking for charges you did not make; and (2) your credit report. You can request your credit report free of charge on the web at www.annualcreditreport.com, or by calling toll-free 877-322-8228. 
 
    Second, if you believe you are the victim of identity theft you may want to place a fraud alert on your credit reports to let creditors know to contact you before opening a new account in your name. Once you place the fraud alert in your file, you're entitled to order one free copy of your credit report from the credit reporting agency. The fraud alert will remain active for approximately 90 days.  To place a fraud alert, simply call any one of the 3 credit reporting agencies at the phone numbers listed below:

·                     Experian 1-888-397-3742
·                     TransUnion 1-800-680-7289
    When you  receive your credit report, review it carefully for accounts you did not open, debts on your accounts you can't explain or for inquiries from creditors that you did not initiate.  Review your personal information, such as home address and Social Security Number for accuracy. If you see anything you do not understand, call the credit agency or creditor at the telephone number on the report.  Close the accounts that you know, or believe, have been tampered with or opened fraudulently.  You may also work with the credit agency to place a "credit freeze" on your credit files.  
    Third, if you find any suspicious activity on your credit reports, you can file an ID Theft complaint with the Federal Trade Commission online at www.ftc.gov, or by calling 1-877-438-4338.  You may also want to call your local police or sheriff’s office and file a police report of identity theft.  Get a copy of the police report.  You may need to give copies of the police report to creditors to clear up your records.  Also please notify the pension office toll-free at 888-856-3447 to assist our continuing work with the Oklahoma City Police Department. 
                     Even if you do not find any signs of fraud on your reports, we recommend that you check your credit reports periodically. Rest assured that the OFP is diligently pursuing all courses of action to retrieve the confidential information and to protect its members.  If there is anything that the OFP can do to assist you, please call us toll-free at 888-856-3447.
